Job Description:

• Performs screening, evaluation, assessment, program planning, intervention treatment and provides home instruction programs for out-patients and in-patients. 
• Responsible in accomplishing occupational therapy paperworks such as initial evaluations, daily patient notes, progress reports, re-evaluation and discharge plan. 
• Performs other administrative duties such as updating of patient census, rendering order of payment, receiving patient referrals, requesting of supplies at central supply room.                  
• Contributes ideas, suggestions for deliverable and other unit activities. 
• Maintains order and cleanliness of office, treatment area and therapeutic equipment. 

Minimum Qualifications:

• Bachelor's degree in Occupational Therapy
• Four (4) hours of relevant training
• One (1) year of relevant experience
• RA 1080 (Occupational Therapist)

Salary:

• PHP36,619/monthly
